Prenatal massage comfort: positioning, pillows and speaking up

A position is a starting point to adjust, not a guarantee of safety.

The short answer

What to remember

  • Ask your maternity provider whether the session is appropriate for you.
  • Side-lying and pillows can be discussed and adjusted to your needs.
  • Stop for concerning symptoms and seek appropriate care.

Comfort starts before positioning

No pillow arrangement can guarantee that massage is safe for every pregnant person. Before booking, ask your maternity care provider whether a wellness session is appropriate for you and whether there are restrictions on movement, pressure or areas of the body. If your circumstances change before the appointment, check the advice again rather than relying on an earlier conversation.

Choose a prenatal service and ask whether a trained therapist and suitable equipment are available. A clear plan helps the therapist prepare for your visit, while medical advice remains specific to you. You can postpone or stop if your condition or comfort changes, even after you have made arrangements at home.

Discuss side-lying support

ACOG identifies lying on your side rather than facedown as the best position for massage during pregnancy. Discuss support for your head and the use of pillows between your knees or near your belly, guided by your clinician’s instructions and your own comfort. The exact arrangement can be adjusted; a picture of a setup is not a prescription.

Give yourself time to settle before the session starts. You may prefer a pillow nearer or farther away, a different side or a short break. Ask the therapist to adjust the support before continuing. Move slowly when changing position and avoid staying still just because the session has already begun.

Check the setup before touch

Once positioned, take a moment to breathe and speak normally. Notice whether there is uncomfortable pressure on your belly, a pulling feeling around your pelvis, or awkwardness in an arm. Describe what you feel to the therapist, even if the adjustment seems small. A comfortable start does not mean that the same setup will remain comfortable throughout.

Wear clothing that is easy to adjust and agree on draping and privacy in advance. Tell the therapist which areas you do not want touched. You can ask what will happen next, change a boundary or request a pause at any time. A towel or pillow never replaces your consent.

Keep the conversation open

Explain your preferred pressure and the relevant instructions from your clinician. Simple phrases such as ‘lighter, please’, ‘I need to change position’ or ‘stop now’ are enough. A wellness massage is not a test of endurance, and painful pressure is not a measure of a successful session. Check in whenever your comfort changes rather than waiting until the end.

Ask for water, a bathroom break or a moment to rest whenever needed. Keep essentials within reach and allow time afterward to move at your own pace. Vela Spa can receive your preferences when you arrange the visit; during the session, continue telling the therapist what works for you.

Stop and respond to symptoms

Stop the massage if you feel dizzy, uncomfortable, breathless or experience pain, and seek the appropriate medical assessment. Do not assume another pillow will resolve a symptom. The CDC identifies chest pain, trouble breathing, severe persistent headache, pregnancy bleeding and leaking fluid as urgent warning signs requiring immediate medical care.

Contact your maternity team about a concerning change in your baby’s movements or another symptom that feels wrong. After a comfortable visit, note the support and draping you liked for a future discussion, but reassess if your pregnancy changes. What felt right on one day may not suit the next appointment.

Quick answers

Frequently asked questions

Does side-lying guarantee safety?

No. It is a position described by ACOG; your own medical advice and circumstances still matter.

Must I stay on one side?

Discuss changing position or taking a break with your therapist, following your care provider’s guidance.

Can I decline touch on an area?

Yes. Set boundaries before the session and change them whenever you need.

What if I feel dizzy?

Stop the session and seek appropriate assessment rather than assuming a pillow adjustment is enough.
